Dr. Cohen accommodates and treats most private insurance patients as well as Workers' Compensation patients. 

• Please bring your insurance card and a photo I.D. with you to your first visit.

• Your insurance will be verified prior to your appointment.

• We bill your insurance company. Payment is due at the time of service. At your request, an estimate can be provided prior to services being performed.

• Methods of Payment: We accept Cash, Check, Visa and Mastercard.
