YOUR FIRST VISIT

Arrival Time

To allow time for check-in and registration, please complete new patient paperwork online before arriving to the office.

Please arrive at least 20 minutes before your scheduled appointment.

If you haven’t completed online paperwork in advance, please arrive 45 minutes prior to your appointment so that our staff has time to help you complete it. You’re encouraged to bring a family member or friend to help with paperwork if you are unable to complete at home - and our staff is glad to assist as well.

If you are running late please contact the office in order to accommodate you and other scheduled patients. ay be asked to reschedule so that everyone receives the full time they need with Dr. Cohen.

An insurance card and a driver's license are required for identification.

If you were treated by another physician for the same problem, it is very helpful to obtain the following documents prior to your visit:

Documentation of previous treating physicians / surgeons
All x-rays, CAT Scans, MRI's and/or lab tests
Documentation of previous therapists

If you have splints, please bring them with you.
